| Cardiac Care |
Our Cardiac Care staff has extensive experience in cardio-pulmonary care and rehabilitation. They provide specialized services to patients with a wide range of acute and chronic cardiac conditions. The Cardiac Care staff is part of an interdisciplinary team that includes community health nurses, medical social workers, Home Health Aid, and physical, speech and occupational therapists.
The cardiac care staff collaborate with physicians to develop and implement the most effective care plan for each patient.
- Assessment of patient’s physical and mental health
- Ongoing assessment and evaluation of the care plan
- Home EKG
- Point of care testing for anticoagulation management and congestive heart failure
- Telemonitoring
- Weight monitoring
- Monitoring of heart rate and blood pressure pre- and post activity
- Oxygen saturation monitoring
- In-home respiratory therapy instructions
- Patient and family teaching and guidance
- Nutrition counseling
- Interdisciplinary conferences with physicians and other health team members
- Referral to and coordination of community resources and ancillary programs
- Specially trained Home Health Aid to assist with personal care needs
- Private duty services through private duty affiliate, CareSource
