| Medical Social Work |
Medical social workers are fully – integrated members on our interdisciplinary team. They work closely with our nurses, therapists and Home Health Aid. Their specialized skills address the social, economic and emotional issues relating to illness or injury and mitigate the impact on an individual’s medical condition, treatment plan or rate of recovery.
Our licensed clinical social workers are highly experienced in:
- Community health
- Family systems
- Human development
- Aging
- Mental health therapy
- Community resources
- Entitlement programs
- Long-term care placement
Social Work Interventions include:
- Assessment of psychological functioning
- Short-term counseling for patients and families to help them adjust to illnesses and altered lifestyles, as well as to help strengthen family support systems
- Crisis intervention for clients whose safety is compromised
- Counseling to assist clients to access information for government agency social services
- Stress reduction counseling for primary caregivers
- Individual treatment, counseling and family referrals for domestic violence victims
- Assistance with placement to extended care facilities, assisted living facilities and other supportive living environments
- Patient and family education and support
- Interdisciplinary conferences with physicians and other health team members
- Referral to and coordination of community resources and related programs
