Our Maternal/Child Health Team includes registered nurses with a foundation in community health and experience in routine and high-risk obstetrics. Our nurses assess each patient's needs and provide access to other community-based services to help mothers-to-be through pregnancy to term. Postpartum visits are available to mothers and infants who have been discharged early or at high-risk. The team also includes:
Behavioral health nurses
Medical social workers
Home Health Aid
VNA/SCC professionals work in close collaboration with physicians to develop and implement the most effective personalized care plan for each patient.
Routine visits include:
- Physical assessments
- Birth and infant care guidance and education
- Assessment of fetal heart rate
- Medication teaching and administration
- Referral and coordination of community resources and related programs
Our team manages complex obstetric conditions such as:
- Hypertensive disorders in pregnancy
- Gestational diabetes
- Preterm Labor
- Hyperemesis
- Intrauterine growth retardation
- Pre-existing medical conditions
- High-risk social situations: substance abuse, teen pregnancy
- Postpartum depression
- Sudden Infant Death Syndrome/fetal demise
