We offer state of the art Wireless Telemonitoring, which enables patients to monitor their blood pressure, oxygen levels, heart rate, weight, blood glucose and hemoglobin A1c from the comfort of their home. The data collected via the telemonitoring system is sent to physicians over the phone.
After the patient data is sent, the patient picks up the telephone and an interactive voice response system tells them their recent readings, and congratulates them if they meet their goal. The interactive voice response system also asks the patient questions about their symptoms and medication compliance. The results then appear on the patient reporting website and are available to our home care agency as well as to the physician. The data collected is monitored by a nurse and can be sent to the doctor via fax or telephone call. A nurse follows-up with the patient after each day’s reading and visits the patient at home when necessary.
For each patient referred to the VNA of South Central CT, we can provide:
- Assessment to ensure that the patient is appropriate for telemonitoring
- Daily monitoring of vital signs by a nurse
- Communication with health care providers of monitored results are not within normal limits for the patient
- Reassurance for the patient that a nurse is monitoring their health status on a daily basis
